The Patient Care Coordinator, with the supervision, support and guidance of clinic physicians, supports the development, implementation and success of the Patient Care Coordination program by working with patients, staff and other local support teams. The Care Coordinator will serve as the primary interface between the clinic, individual primary care physicians and their patients and health plan sponsors.
The successful candidate will possess flexible thinking, excellent communication skills, the ability to examine, evaluate, and discuss the effectiveness of the care plans. He/she must be flexible and have the ability to work collaboratively with physician, clinic staff and patients, be dedicated to their quality of work, and diligent about performance monitoring.
The successful candidate will understand and have a commitment to the philosophy, mission, values and vision of the clinic. The qualified individual will be able to demonstrate these values with his/her leadership practices and a personal commitment to health and well-being. This individual will also possess both a professional integrity and demeanor, and be a professional representative of the clinic.
